  • In case we need to contact you for follow-up questions about your nomination
  • The name of the athlete you would like to nominate
  • What sport or sports does your nominee participate in?
  • Explain why you think this athlete is deserving of special attention. For instance, did he or she: break records, make game winning plays, step up as a team leader (i.e. team captains, but doesn’t have to be), support the team in a unique way, make/start on varsity teams as a freshman? Tell us what makes this athlete special. and many more impressive things
